DEFINITION:
In order to ensure timely processing of the patient record, the HIM Technician will ensure receipt of all inpatient/outpatient/emergency department patient records; review each patient record for poor original, items that cannot be scanned, patient identification on each document, and will group like documents in the appropriate order. The HIM Technician is responsible for scanning all documents/records into Horizon Patient Folder on a daily basis. This position will maintain daily workflow, insuring that records are scanned in a timely and efficient manner to avoid delay in further processing. This position is also responsible for cleaning and maintaining scanning equipment. The HIM Technician is responsible for assuring that all patient records and loose documents are scanned into Horizon Patient Folder with the highest level of quality possible. This position is also responsible for indexing all documents to the appropriate patient folder and for ensuring each document is assigned the appropriate document name. In order to allow for timely completion of the medical record, the HIM Technician will review each discharged patient's record for incomplete or missing items, identify the responsible practitioner for each of these items, and link the deficiencies to the appropriate practitioner for completion. The HIM Technician will be responsible for verifying chart completion and monitoring the reanalysis queue to ensure all records are completed appropriately. The position will also be responsible for timely notification to practitioners of pending records that need completion and identifying those practitioners who will be suspended. The HIM Supervisor/Manager will be notified accordingly. The position will be responsible for answering the telephone and routing calls; assisting customers with record requests, pull lists and general questions; reviewing requests for release of information; preparing and transmitting baby birth and death certificates; collecting information required by health department; assisting transcriptionists and coders; and identifying and performing other duties as needed.
PREFERRED QUALIFICATIONS:
High school diploma or G.E.D. with preferred courses in typing, filing, office methods and computer skills.
